If something we made didn't sit right with your skin, we want to know. This is the form for it. It takes a couple of minutes and a real person reads every single one.

First, look after yourself

  1. Stop using the product.

    Set it aside and keep it — jar, label, lot number and all. If we need to check a batch, that container is the evidence.

  2. If it's serious, get medical help right now.

    Trouble breathing, swelling of the face, lips, mouth or throat, chest pain, fainting, a rash spreading fast, or anything that frightens you: call 911 or go to an emergency room. If someone swallowed a product, call Poison Control at 1-800-222-1222 (free, 24 hours, anywhere in the US). Please don't wait on us first — we're two hands in a Landrum kitchen, not a medical service.

  3. Then come back and tell us.

    Once you're safe and steady, fill in the form below. Nothing here replaces seeing a doctor; it just makes sure the shop knows, on the record.


Tell us what happened

Federal law (the Modernization of Cosmetics Regulation Act) says a body-care label has to carry a way for you to report an adverse event. This page is ours. Fill in what you know — blanks are fine, and “I don't remember” is a real answer. Only your email and a description of what happened are required.

We send your reference number here. It's the only field we truly need besides the description.

The FDA's form has boxes for age and sex, which is the only reason they're here. “Prefer not to say” is a perfectly good answer and costs you nothing.

What you send here is kept on file for at least three years and shared with the FDA only when the law requires it. Details are in our privacy policy.


What happens after you hit send

  • You get a reference number on this page and by email, straight away.
  • Savanna reads it. Not a queue, not a bot — the person who made the batch.
  • We check the batch. If a lot number points at something, we pull that lot and contact anyone else who bought it.
  • Serious events go to the FDA within 15 business days, on the MedWatch form (FDA 3500A), as the law requires. You don't have to do anything for that to happen.
  • We keep the record. Federal law says a shop our size holds a reaction report for three years. We don't delete them on a timer, so in practice it's longer — a record you still have is never the problem.

You can also report an adverse event directly to the FDA yourself, whether or not you tell us — search for “MedWatch” on fda.gov. Telling us doesn't sign anything away, and it doesn't stop you doing that.

Every ingredient in every batch is listed on the product page and on the label. If you know you react to something, read those first — and if a label is ever unclear, that's on us. Tell us and we'll fix it.
